Parent/Guardian Consent for Marketing Purposes

By checking this box, I give permission for STEAM Scholars Tutoring to use photographs, videos, or other media of my child in marketing materials, including but not limited to social media, brochures, and the company website. General Participation Consent

I consent to my child participating in the STEAM Scholars Tutoring workshop, including all activities provided as part of the program.

Emergency Medical Consent

I authorize STEAM Scholars Tutoring staff to seek emergency medical treatment for my child if necessary and understand that I will be notified as soon as possible. I agree to assume any associated costs.

Liability Waiver

I release STEAM Scholars Tutoring, LLC and its staff from any liability for injuries or accidents that may occur during the workshop, except in cases of gross negligence.

Data Privacy Consent

I consent to STEAM Scholars Tutoring collecting and securely storing my personal information for the purpose of enrolling my child in this workshop. I understand my information will not be shared with third parties without my explicit consent.

Behavioral Expectations Consent

I acknowledge that STEAM Scholars Tutoring has established behavioral expectations for all participants to ensure a safe and positive learning environment. I understand that failure to comply with these expectations may result in my child being asked to leave the workshop.
